The serious collision between a red Vauxhall Astra and a red Nissan Micra happened on the A365 Bath Road in Melksham at about 14:10 GMT on Monday.
Wiltshire Police said the Nissan, driven by the woman in her 40s, was thought to have been stationary and turning right onto Bowerhill Lane, when the Vauxhall collided with it.
The driver of the Vauxhall, a man in his 30s, was uninjured.
The road was closed for several hours while enquiries took place but has since reopened.
